1.6 Fill out Online Student’s Data (SPIS)

1.7 Secure Pre-Enrollment Form

2.1 Submit Pre-Enrollment Form

2.2 Obtain Subject Assignment / Student Enrollment Load Document

 

Step 3 - Assessment

3.1 Secure Signed Pre-Assessment Form & Payment Plan

 

Step 4 - USC

4.1 Obtain Assessment of Institutional Fees

 

Step 5 - Cashier

5.1 Pay Tuition and Institutional Fees | Payment Online: https://uphsl.edu.ph/online_payment/guest.php

 

Step 6 - Chancellor

6.1 Confirm Payments

 

Step 7 - Registrar

7.1 Submit Pre-Enrollment Form

7.2 Secure Official Registration Form

 

Step 8 - SPS

8.1 Apply for an Identification Card (ID)

8.2 Measurement of Uniform

 

Step 9 - Medical & Dental Clinic

9.1 Undergo Medical and Dental Exam
