• Letter of Intent to Enroll

• Copy of Authenticated Documents w/ Red Ribbon or Apostille from the point of Origin

  • Scholastic Record

  • Affidavit of Financial Support

  • Birth certificate

  • Police Clearance (Local Police Office)

  • Medical Certificate

  • Marriage Certificate (if applicable)

• Passport Copy & ACR (Alien Certificate of Registration)

• Three (3) pcs. Recent 2 x 2 Picture (White background w/ name)

• Certificate of Transfer Credentials (for Transfer Student)

• CHED Endorsement (for Transfer Student)

• Certificate of Dual Citizenship (if the student claims dual citizenship)
